To register your child for camp, please print out the following Camp Application & Liability Waiver. We also need a copy (front & back) of the medical insurance card. After filling those out you can either mail or fax the information to us along with your credit card information or a check or money order.
[Liability Waiver] This form should be completed for ALL campers
[FLOC Liability Waiver] **This form only needs to be completed for those participating in the Overnight Adventure Weeks
[ACE Liability Waiver] **This form only needs to be completed for those participating in the Multi Adventure Overnight Week
[Wilderness Voyageurs Liability Waiver] **This form only needs to be completed for those participating in the Multi Adventure Overnight Week
Payment Schedule
Remaining balance due May 15th. Reservations after May 15th must be paid in full at time of reservation. Price includes all activities. 6% West Virginia sales tax and a $10.00 per camper per week in user fees will be added to the price of each camper.
